The purpose of the Armes Family Cancer Care Center Social Worker is to assist in addressing the psychosocial and financial needs of the diverse patient population served at The Armes Family Cancer Care Center. The Social Worker is to identify and help reduce patient financial burden through the utilization of pharmaceutical and hospital assistance programs, as well as national and local resources. Additionally, through the evaluation and monitoring of the oncology office’s self-administered psychosocial distress screening tool, this position is able to identify and intervene on behalf of the patient and caregiver. The Social Worker will identify and offer psychosocial support during pivotal times in their care and coordinate additional community resources, as needed. JOB DUTIES/RESPONSIBILITIES Duty 1: Work with all patients at The Armes Family Cancer Care Center to insure maximum utilization of available financial coverage/ funding sources for patients including, but not limited to, Medicaid, Medicare, HCAP, Affordable Health Care Act, and pharmaceutical drug assistance programs. Duty 2: Conducts insurance verification via phone, fax and online methods for Cancer Center services and responds to patient and family questions and concerns regarding insurance benefits. Duty 3: Works with patient financial services to inform patient of payment options and identifies potential for bad debt. Provides patient with an estimated cost of services when possible Duty 4: Applies “financial awards” payments to patient accounts from pharmaceutical company programs utilizing data online or BV Cashiers office Duty 5: Establishes and maintains communication/ collaboration with the interdisciplinary team at The Armes Family Cancer Care Center regarding patient psychosocial condition, and anticipated needs to ensure exceptional patient care. Duty 6: Acts as a liaison between the facility and patient/ family to resolve problems and address complaints. Has strong focus and commitment to Service Excellence and Patient satisfaction. Duty 7: The Utilizes educational materials and outside resources as needed, through the coordination of patient care with community agencies. Duty 8: Facilitates interventions for the patient/ family making personal, ethical and legal decisions to manage patient care on an as needed basis. Duty 9: The Social Worker is responsible for ensuring compliance to Commission on Cancer Standard 3.2, Psychosocial Distress Screening. This is done through process evaluation, monitoring, and reporting findings and outcomes as appropriate to needed team members and the cancer committee. Duty 10: Advocates on behalf of the oncology community by participating in various committees and meetings for the purpose of improving patient care and outcomes. Duty 11: Performs all duties and responsibilities in a manner consistent with and supportive of the mission and values of Blanchard Valley Health System. |
