Job Description: Arizona Licensed Social Worker
MSW Social Worker for aging population. Job includes Chronic Care Management Supervisor to assess and direct Case Managers, and Fitness Staff to monitor and provide “preventative,” health related services for aging/disabled members with two or more chronic conditions. This program is part of a Fitness Center that is open to the public with a large percentage of senior citizen/ handicapped members.
A Social Worker for a Comprehensive Outpatient Rehabilitation Facility, to work with other staff, patient’s families, and caregivers. Patients are referred to this facility to receive physical and psycho-social services in an effort, to maximize their independent living skills and to communicate/coordinate services between their referring physician(s) and community-based services.
These programs are located in the same building with access to an indoor pool, jacuzzi, sauna, weights/cardio gym, offices and activity rooms. Wages and benefits and are commensurate with employee’s work needs, experience, and work ethic. Forward questions and resumes to:
