Contract Responsibilities Summary

  • Psychopharmacological care 
  • Prescribing medication and documentation of medication changes and additions 
  • Performing initial evaluations and follow up evaluations 
  • Enter into NYAPs EVOLVE system all psychiatric evaluations and appointments, including follow-up appointments, for each session of direct service performed. 
  • Documentation of services 

Requirements

  • MD
  • Valid licensure in accordance with state of Illinois standards and regulations
  • Flexible availability to meet clients needs: estimated 5-15 hours weekly
  • Experience conducting telehealth services preferred
  • Proficient with technology, computer systems and databases
  • Excellent communication skills both written and verbal
