 MedMark Services is looking for an empathetic, persuasive and cooperative Licensed Clinical Social Worker(LCSW) to case manage work at the VA Primary Care Outpatient Clinic. The Social Work Case Manager will perform a variety of functions including mental health therapies, diagnostic procedures, and service coordination.

Location availability:  Fort Pierce and Stuart

Essential Duties & Responsibilities:

• Conduct psychosocial assessments and identify interventions to optimize patients’ functional status, well-being, and access to care.
• Coordinate community-based services including transportation, supportive home services, skilled nursing home placement/long-term care for eligible veterans; provide information and referral to state and community agency programs; assist with resource linking.
• Provide patient and community education; brief supportive counseling; mental health triage as needed.

• Must hold current LCSW in state of Florida
• MSW from a school of social work fully accredited by the Council of Social Work Education
• 3 years’ post-graduate social work experience
• Demonstrated critical thinking skills, initiative, ability to work independently with strong patient-centered focus, clinical documentation skills, experience with electronic records
• Current Basic Life Support (BLS) certification preferred
• Work in previous healthcare setting preferred
