CCS Children's Service Facilitator

Working with our Manitowoc County Human Services Department team, this position facilitates services for children and adolescents who have been diagnosed with a mental illness and provides crisis services during work hours one day per month and as needed. This position:

• Completes comprehensive bio-psycho social assessments and functional screens.
• Acts as service facilitator by developing teams of formal and informal supports, collaborating with schools, parent's, foster parents and service providers, developing service plans and coordinating contracted services.
• Develops crisis plans and provides crisis services.
• Facilitates referral, admission, transfer and discharge arrangements with inpatient and residential facilities and providers.
• Generates and maintains clinical records and reports per state standards and agency policy

A Bachelor's Degree in Social Work or closely related field is required. Must have state Social Worker Certification within six months of hire. Prior experience working with chronically mentally ill is required.
