Position Overview

Summary

The Bilingual Foster Care Case Manager works to make a meaningful difference in the lives of the families being served by the National Youth Advocate Program by working closely with the youth and family to provide advocacy and support towards targeted goals. 

 

Responsibilities

  • Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies 
  • Participates in conferences with agency personnel and the family in the home. 
  • Completes or revises the Family Risk Assessment with all family members that live in the home to assess the family’s strengths and needs, as well as risk of harm to the child/children in the home. 
  • Participates in family centered meetings. 
  • Prepares for and participates in administrative and court reviews of the case plan. 
  • Provide in-home supportive services: parenting education, counseling, community resource linkage and referral, advocacy and other social service intervention to families and children. 
  • Maintains contact with families through regular and planned, as well as unannounced, visitation of parents and children. 
  • Prepares and participates in court proceedings when child placement out of the home becomes necessary. 
  • Works with Foster Care Programs to coordinate visits with the Foster Treatment Coordinator in the Foster Family’s home to improve communication. 
  • Pursues permanent custody and planning for adoption or other permanent substitute care for children who cannot be returned to their birth families. 
  • Performs duties on-call as outlined in on-call description when called upon to respond to after hour emergency situations. 
  • Attend weekly supervision meetings and actively participate in the supervision process. 
  • Makes immediate reports to the supervisor of incidents involving risk of harm to a child/children or mattes of concern affecting agency function. 
  • Participates in team meetings, clinical problem solving, and review meetings. 
  • Utilize Children Welfare Best Practice Principles in decision making. 
  • Performs other duties as assigned. 
  • Completes timely Record of Activities as well as monthly case assessments and summaries. 
  • Completes Case Plans within thirty days of the case openings including obtaining all parties signatures or documents reason for lack of agreement/participation. 
  • Amend Case Plans according to agency policy. 
  • Prepare semi-annual reviews according to agency guidelines. 
  • Prepares complaints, affidavits, motions, and annual review reports for Juvenile Court. 
  • Completes and updates timely individual Child Care Agreements and Education/Medical consent forms for children placed out of their homes. 
  • Keeps family case record documentation properly organized and filed according to HIPPA, COA, and agency policies. 

 

Minimum Qualifications

  • Bachelor’s Degree in Social and/or Behavioral Science 
  • Bilingual: Spanish/English fluency
  • Experience in the delivery of social services in a diverse cultural population with infants, children, and adolescents
  • Reliable transportation 
  • Driver’s License with a safe driving record 
  • Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs
