Position Overview
Our team is hiring Case Workers for a new special program. The focal goal for this new program is to verify any sponsor (parents, guardians, or family friends to whom the UAC will be released) is capable of providing for the child’s physical and mental well-being and that the sponsor has not engaged in any activity that would indicate a potential risk to the child by responsibly conducting activities to include but not limited to culturally sensitive home study investigations, assessments, referrals, case management services, documentation of required reports, case notes, and case contacts into the appropriate database in compliance with contract terms or tailored to the client’s needs, expected timeframes and agency expectations defined by your direct supervisor.
Summary
The Case Manager works to make a meaningful difference in the lives of the children being served by the National Youth Advocate Program by working closely with the youth and family to provide advocacy and support towards reunification.
Responsibilities
The Case Manager will perform duties including, but not limited to:
- Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.
- Coordination of the completion of assessments of people served.
- Completion of individual service plans.
- Assessment of potential sponsors and manage communication regarding child’s progress.
- Make transfer and release recommendations under the guidance of the Lead Case Manager.
- Coordinate the release of children and/or youth from NYAP care.
- Provide direct assistance on completing sponsor application packets and ensuring supporting documentation.
- Ensure that information gathered is shared with appropriate staff and stakeholders during sponsor assessment process.
- Provide weekly status updates to external stakeholders regarding progress of achieving safe and timely release.
- Inform other stakeholders (local legal service providers, attorneys of record, other local and/or national service providers and/or federal agencies) of the progress of the child’s case.
Minimum Qualifications
- Bachelor’s degree in education, psychology, sociology or other relevant behavioral science field.
- Two (2) or more years of demonstrated experience in social work and basic principles, concepts and methodology of social work.
- Excellent written and communication skills.
- Experience in the delivery of social services in a diverse cultural population with infants, children, and adolescents.
- Reliable transportation.
- Experience working with immigration or resettlement programs preferred.
- Knowledge of state, community and agency resources for clients.
- Driver’s license with a safe driving record.
- Bilingual English/Spanish fluency.
- Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.

Work Schedule - Due to the importance of this position, employees supporting this contract can be required to work extended hours to include evening work, on-call, and weekends to support time-sensitive or real-time complex services. This position is considered ESSENTIAL.
This means that Case Workers are required to work during emergency situations, including inclement weather, natural and man-made disasters, etc.
