LMHP preferably with substance abuse or CAC, LAC experience. Provides mental health assessments, therapeutic services, crisis intervention, and/or any other identified clinical services deemed necessary to stabilize and improve the functioning of individuals served. Licensed Mental Health Practitioner provides services to children, their families and/or adults.
ESSENTIAL JOB FUNCTIONS AND REQUIREMENTS
Individual and/or group counseling
Group Psychosocial Skills Training
Participate in quality improvement and staff training activities
Implementation of behavior management plan in the home, school, office, and/or other environment.
Provision of parent education training both, individually and in groups.
Ongoing assessment of clients’ needs.
Provision of Crisis Intervention Services to the client and/or family.
Provision of consultation and direction to school personnel and other support to the client (i.e. after school personnel, sport coaches, etc)
Linking clients to needed services and advocating for clients as needed
Participation in clinical meetings, staff meetings, and staff trainings.
Responsible for development of ISRPs, implementing the treatment plan and monitoring the progress of each assigned client.
Conduct or participate in case reviews and/or case staffings as required.
Maintain up to date client records and complete all service logs in a legible, timely and accurate manner.
Responsible for reviewing all client records on caseload to include reading the assessment, ISRP, psychiatrist notes/evaluation and all other pertinent clinical information.
Responsible for ensuring the timely completion of all mental health rehabilitation forms and necessary documentation to include the assessment, ISRPs, reassessments, service logs and discharge notes.
Attend supervision 2 hours weekly for the first ninety days and one hour monthly thereafter.
Attend weekly staff meeting
Review case referrals and share client information with Clinical Director when cases are referred or assigned.
Responsible for the timely completion and submission of all time sheets, service logs to the Clinical Director for each pay period.
Serves as an agency liaison by maintaining collaborative efforts to ensure that timely and quality services are provided to referred clients.
Appears in court when subpoenaed as a witness.
Completes ISRPS, reassessments every 90 days and submits documentation 14 days prior to PA expiration date.
Responds to or arranges for a response to calls for families in crisis on a 24 hour, seven days a week basis, if applicable.
Meets or exceeds individualized productivity expectations.
Participates in the agency’s Continuous Quality Improvement process as requested.
Meets all agency and licensure training requirements on an annual basis.
Education and Experience: Master's degree from an accredited university or college with a major in the field of counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field.
Two years professional experience providing services to persons with serious mental illness.
Licensed as an LPC or LCSW in the State of Louisiana and adhere to all licensing board requirements to remain in good standing.
