SUMMARY OF POSITION: The goal of this position is to provide outcomes-driven case management services to the reentry population and their families, working in collaboration with our partners. This full-time position (40 hours/week) will report to the Staff Supervisor and work from an office in Los Angeles.
DUTIES: This individual must work successfully with all persons without regard to ethnicity, religion, age, national origin, physical challenge, sexual preference, or gender.
Conduct eligibility process. Complete assessments for qualifying fathers through which an Individualized Case Plan will be created.
Meet weekly with clients to evaluate client needs, provide linkages to additional resources, coordinate services and provide personal support.
Maintain client records and complete case notes; logging events and progress.
Conduct follow-up services (to Academy alumni) to determine whether each client’s needs are being met and to determine the outcomes of services.
Facilitate curriculum-based workshops as requested.
Assist with Outreach and Recruitment, as requested.
Assist with the facilitation of evening Support Group sessions.
Assist with the facilitation of occasional weekend, family events.
Work collaboratively with all program staff, co-located staff, and linkage organizations and agencies.
Create and maintain complete, accurate, and orderly client files in confidential storage.
Prepare and provide program data on a scheduled basis to the Data-Entry Specialist.
Participate in case conferences, weekly supervision sessions, case management meetings, and staff meetings.
Participate in trainings and community and other meetings as requested by the Supervisor.
In cooperation with other program staff, meet or exceed contractual goals.
Bachelor’s Degree in Social Work or a related field from a certified college/university (required).
3 years of case management experience with multi-barriered populations.
Certification/training in domestic violence, parenting, trauma-informed practice.
Familiarity with the criminal justice system a plus.
Experience working in a collaborative setting a plus.
Proficiency in Microsoft Office or similar program; Internet; Email, data entry and periodic reporting
Mission-driven person with a strong work ethic, well-organized, reliable, flexible, and upbeat.
Excellent written and spoken English skills. Spanish-speaking a plus.
Reliable vehicle and maintain a valid driver’s license and automobile insurance per California state law.

Services for the “re-entry” population began in 1978. FOLA is the oldest organization of its kind in Los Angeles County. All services are offered at no cost to clients.
