SUMMARY The Program Coordinator will be responsible for assuring program quality for Advance Care Planning. This role will also be responsible for program development, implementation, education, outreach, and outcomes regarding advance care planning across the continuum.
EDUCATION Bachelors degree in Nursing, or Masters degree in Social Work
LICENSURE/CERTIFICATION Registered Nurse in the State of Illinois, or Licensed Clinical Social Worker (LCSW), or Licensed Social Worker (LSW), and Drivers License, and Proof of Auto Insurance.
EXPERIENCE Five (5) years of experience in healthcare, program/committee planning, outreach, metrics, process improvement, chronic disease, education. Formal leadership experience preferred.
OTHER SKILLS Leading teams, streamlining operational processes. Track record for being results-oriented, demonstrated ability to drive performance and outcomes. Highly organized and excellent relationship/interpersonal skills.
