Clinkscales Elder Law Practice is seeking a full-time professional Care Coordinator for clients in the region covering McPherson, Lindsborg, Salina, Hutchinson, and Wichita areas. Coordinate care for clients and families dealing with chronic health and/or aging issues; provide client care assessments, monitoring and education to clients and families. Assist families in understanding care options and assistance programs; counsel families and make recommendations. Represent and advocate for the client with doctors, attorneys, CPAs and other care providers. Travel one to two days per week to meet with clients in their home or other setting is required. Home office can be located anywhere within the territory.
A degree in Social Work or Nursing is required, with an emphasis in Geriatrics preferred; at least two years’ experience in health care is required. Must have a valid driver’s license.
