Community Reach Center (CRC) is an Integrated Health Care and Trauma Informed Care Community that serves a metropolitan area in Denver, CO. Our 500 employees offer a wide variety of services to individuals and families in schools, day-treatment, residential, hospital, and correctional facilities.
We are seeking a LCSW Therapist to join our Thornton Outpatient Team who is committed to enhancing lives every day. How will a LCSW Therapist be successful in this role? Under direct supervision, provides behavioral health consultations, clinical and support services to consumers and/or their families using evidence based practice models. May operate in a residential environment, or in an outreach environment, such as in homes, schools, outpatient clinic or primary care site.
Provide effective treatment planning and assisting clients in coordinating care with primary care providers.
Evaluate crisis situations and implement appropriate interventions.
Provide on site assessment, consultation and brief intervention for psychological and psychiatric problems and disorders in a manner that promotes safety, comfort and the therapeutic environment.
Teach patients, families and staff the care, prevention and treatment techniques of psychiatric disorders.
Assist the primary care team in developing and implementing care management processes, such as the use of guidelines, disease management techniques, case management and patient education.
Minimum of a Master’s Degree in a counseling related field or a Doctorate degree in Psychology from an accredited university;

The Center serves more than 10,000 clients each year at five outpatient locations throughout Adams County, as well as in schools, nursing homes, its own residential treatment homes, physicians' offices, emergency rooms, private residen...ces, and jails.
