PCG’s Human Services practice area has an excellent opportunity for a Social Worker to provide intensive case management services to families participating in the CalWORKs program, with the goal of employment and achievement of higher levels of self-sufficiency. This position has been created to support the Wrap Around Services Program, a pilot program combining intensive case management provided by social workers, client mentoring provided by seniors age 60 and above, and coordination of community supports and services. The social worker will carry a caseload of approximately 20-30 clients at any given time, and work collaboratively with the client’s Senior Mentor and Employment Training Advisor (ETA).
Assist in the development of program design and provide recommendations for tool/s to be used to assess client strengths and barriers.
Provide closely supervised intensive case management assistance to promote collaborative and supportive relationships between the family and other support systems.
Perform intake and provide assessments to identify strengths, barriers, and social service needs of individuals and families.
Organize and coordinate treatment services such as Welfare-to-Work case management, behavioral health providers, school personnel, community partners and other government entities (as necessary).
Create and utilize effective plans of action to help individuals identify and overcome barriers that have prevented them from being able to gain employment and improve quality of life.
Successfully coordinate all resources needed to achieve effective plans of action and contingency plans for child care and other working parent issues.
Lead regularly-scheduled case conferences also involving Senior Mentors, ETAs, and as needed, the clients themselves.
Document and track assessment results, case actions, and client progress.
Work effectively with Program Coordinator and team of Senior Mentors to strengthen families and provide them with coping skills as they prepare to enter/return to the workforce.

Strong passion for the social work field.
Experience conducting intakes to determine mental health, physical abuse, substance abuse and social service needs of clients and their families.
Experience providing behavioral health services to children, youth and families.
Experience in leading diverse work teams to effectively build program excellence.
Demonstrated ability to establish and execute defined program goals and objectives to ensure compliance measured performance.
Superior organizational and interpersonal skills.
Experience engaging and maintaining relationships with community partners serving program participants.
Exceptional strength in strategic thinking, development of effective action plans, and problem solving.
Proper etiquette including customer relation techniques, superior verbal and communication skills.
Ability to prioritize work and meet deadlines.
Ability to recognize and maintain the confidentiality of all materials in the work setting.
Demonstrated ability to use various computer systems. Familiarity with CalWIN preferred, but not required.
Ability to partner with and motivate clients from a diverse range of backgrounds.
Ability to work independently, requiring minimal supervision.
Required Education and Experience:
A minimum of a Master’s Degree from an accredited U.S. college or university, or a certified foreign studies equivalency in counseling, social work, or a closely related field or doctorate degree in psychology;
AND at least two (2) years of full-time, post master’s degree experience working in a behavioral health program providing counseling and crisis intervention,
Preferred, Licensure by the State of California as a LCSW, MFT or LPCC
Proficient with MS Office: Word, Excel, Outlook, PowerPoint
