HealthEast is currently seeking an experienced full-time Psychotherapist 2 to join our team of mental health providers. This position will be a part of the Karen Chemical Dependency Collaboration (KCDC), a project of HealthEast Roselawn Clinic. Assesses and treats the behavioral, psychological, social, spiritual, and emotional aspects of patients’ needs. Responsible for providing individual therapy, group therapy and chemical dependency treatment at HealthEast Roselawn Clinic to ensure the provision of individualized treatment and quality mental health and chemical dependency services for Karen refugees from Burma. HealthEast Roselawn Clinic serves about half of the 8,000 Karen refugees in the Twin Cities.
Completes comprehensive, culturally-relevant, trauma-informed diagnostic assessments to diagnose, identify needs, and establish initial goals. Completes documentation that includes all required elements as defined by DHS and incorporates information from collateral sources.
Incorporates information from available collateral resources when developing plan of care. Completes timely and accurate documentation of patient encounters per DHS and HE compliance requirements.
Based on the diagnostic assessment develops a comprehensive treatment plan that includes measurable goals and objectives which is updated the every 3 months or as needed. Assists clients/families in adjusting to and decreasing symptoms of mental illness and chemical dependency following the treatment plan.
Makes and communicates care recommendations and determines level of care to patients, physicians, nursing staff and other involved parties. Facilitates completion of the plan of care in consultation and collaboration with emergency department staff.
Provides individual and group therapy and chemical dependency for Karen refugees from Burma.
Completes ongoing consultation with primary care physicians and KCDC staff, and communicates with patient families to ensure understanding of treatment recommendations and anticipated disposition.
Assists client/family in utilizing relevant and appropriate community resources.
Maintains contact with community agencies to promote the exchange of information to facilitate use of these services on behalf of clients.
Actively participates with a team in the development of culturally-specific MI/CD treatment and other community support programs.
Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.
Actively participates in creating and implementing improvements.
Performs other responsibilities as needed/assigned.
Current Minnesota Board of Social Work Licensure as a Clinical Social Worker (LICSW)
Masters degree in Social Work required
Minimum of 1-2 years experience providing mental health services (individual and group) to Refugees highly preferred
Experience working with Refugees and Interpreters highly preferred
