Take your Leadership Skills to the next level - Kootenai Health is looking for a strong, community-minded, and driven leader for a fast growing Social Services department. We are actively recruiting for a Manager, Social Work Care Management.
You will provide clinical mentorship, support, and development to the acute care and ambulatory social work teams. You are responsible for identifying barriers to care progression along with developing and executing timely discharge plans. You will utilize your skills to facilitate processes that connect the inpatient care coordination with the ambulatory care coordination programs. In this role, you will provide strong collaboration of care coordination services with the Kootenai Care Network, ambulatory clinics, and post-acute facilities. Serves as a social work resource to the hospital and case management staff, providing support and direction with complex cases. You are the core leader and will supervise the hospital's social workers and works collaboratively with all areas of patient care to ensure smooth patient transitions. This job will require you monitor, assist with, and improve the clinical work of the hospital social work team while promoting the conceptual, intuitive, and skills necessary for their professional development. While this is a huge opportunity your experience and excellent communication and relationship skills will lend well to this growing service area. Master of Social Work (MSW) from a university accredited by the Council on Social work Education required. Licensed Clinical Social Worker (LCSW) required.
five years’ professional medical social work experience and clinical supervision experience, including qualification as an “approved supervisor” in the State of Idaho or completion of the course within six months of the date of hire.
three years’ leadership experience, working knowledge of mental health and chemical dependency services coordination and a deep understanding of transitional care including insurance requirements.
Hospital based experience preferred.
Demonstrated competence in psychosocial assessments, discharge planning and crisis intervention.
Experience in methods application, casework, supervision and training.
Knowledge and experience with DSM-5 and the application in mental health assessments: crisis management, conflict resolution, and stress management.
