Waikiki Health is now recruiting for a Behavioral Health (BH) Care Coordinator to serve across three of our primary care clinics that serve men and women presenting with substance abuse, SPMI and homelessness. The BH Care Coordinator functions as a member of the multidisciplinary healthcare team in supporting mental health services provided by primary care providers and BH providers. The care coordinator participates in our Patient Centered Medical Home (PCMH), which includes the active management of an assigned panel of high risk BH patients; close contact with patients as indicated for support and the building of treatment compliance; coordination of care between existing providers and other agencies; participation in multidisciplinary patient-centered team meetings; assessing barriers when patients are not meeting treatment goals; managing behavioral health emergencies; conducting behavioral health screenings; facilitating women groups (tobacco, substance abuse, parenting, perinatal, etc.) and implementing brief behavioral interventions during medical visits.
