Licensed Clinical Social Worker
Position Summary: Clinician will assist and guide program members so that they can identify and work toward their life goals. This includes teaching skills, providing case management, and working collaboratively with team members to develop opportunities and to engage program members in active participation.
Responsible to provide client driven psycho-social programming to persons that are challenged by chronic mental illness and substance abuse.
Able to meet same-day expectations for completing documentation.
Monitor symptoms, current mental status, and clinical service needs for program members. Coordinate information with all relevant providers.
Knowledge of and experience in implementing a trauma informed care framework when working with clients, as well as internal and external professionals.
Responsible for carrying a caseload of clients who are certified under 27-65 statutes. Able to meet clinical and administrative expectations associated with 27-65 guidelines. Ability to assess and place M-1 hold if required.
Functions as a colleague and role model working in partnership with program members and other staff in all facets of the program.
Develop mutually agreed upon service plans with recovery focus. Review these plans every 6 months at the minimum.
Facilitate therapeutic, educational and skill building groups. Regularly review and evaluate group curriculum to ensure that group content meets client needs.
Use Motivational Interviewing techniques to engage individuals in treatment and to help them move through the stages of change. Provide case management referral, assessment, treatment planning and monitoring. Attend IDDT team meetings and trainings
Expected to demonstrate customer service focus, good communication and professionalism with client, co-workers and outside agency personnel in order to provide high quality services and to enhance community relationships.
Maintain caseload of individuals struggling with substance abuse and mental illness.
Complete and follow through with financial and clinical information necessary to ensure reimbursement for services provided to consumers.
Participate in a) AllHealth Network meetings and trainings as directed, b) team meetings c) individual and group supervision.
Perform other duties within the scope of the position and within the experience of the employee as may be requested or required.
Education: Master's degree in Social Work. LCSW required.
Experience: Minimum of two years of experience with adults and/or older adults within a human services setting. Prefer experience with substance abuse and mental illness.
Skills: Ability to establish trust and rapport with chronically mentally ill consumers, their families, or significant others. Ability to design and conduct task focused groups. Knowledge of group process.
Other: Ability to negotiate stairs, climb a step ladder, and lift equipment and supplies up to 25 lbs. Must possess own transportation, have a good driving record, a valid driver's license, and proof of insurance.
