Total Care Services, Inc., a non-profit organization, dedicated to serving persons on Medicaid Waivers, is applying for a District of Columbia grant to provide resource coordination services to elderly persons and physically disabled persons, living in the District of Columbia, who receive services through Medicaid. Total Care Services, Inc. seeks professionals licensed to practice in the District of Columbia licensed, to provide Case Management to assist beneficiaries obtain, manage and coordinate services that support choice, independence, dignity, and confidentiality.
Under the grant, beneficiaries will receive support to lead full lives through identifying, linking and coordinating community based supports and services, developed and documented in individualized care plans which reflect person’s strengths, needs, respect, dignity, personal goals, choices and values. Licensed professionals responsibilities will include…
Conducting face-to-face monitoring at least quarterly, and team meetings including lawyers, family members, and stakeholders; coordination with other waiver providers and medical personnel; telephone monitoring; and visits to ensure the individual’s health, safety and optimal satisfaction;
Completion of an acuity scale evaluation within first 30 days of notification that TCS is the provider
Electronic documentation of all contacts with individuals, family members, agencies, health care providers and other stakeholders, in real time;
Developing the IP and comprehensive assessment specifying beneficiary’s preferences, supports and services;
Exploring and accessing alternative supports and/or funding sources to meet identified needs;
Researching eligibility to ensure benefits are active and available;
Problem Solving - Identifies and resolves problems in a timely and responsive manner; uses reason even when dealing with emotional topics.
Professionalism - Approaches others in a tactful manner treating all persons with respect and consideration; accepts responsibility for own actions; and follows through on commitments.
Initiative – Effectively uses independent judgment and initiative
Master’s and/or Bachelor’s degree
Professional license for practice in the District of Columbia
Two or more years Case Management and experience with physically disabled persons and/or the elderly;
Able to effectively execute responsibilities with minimum supervision
Ability to conduct detailed analytical evaluations and document findings; to identify new medical, social, health services, behavioral services, adaptive equipment needs, and other needs as result of monitoring,
Able to communicate effectively orally and in writing, to establish and maintain working relationships, with internal and external professionals
Lawfully authorized to work in the U.S.
Ability to work a flex schedule as needed; good time management skills.
Proficient with Microsoft Office and Excel;
