Job Description

We are seeking a Medical Social Worker to work full time between our Tracy and Stockton clinics. You will provide assessment, case management, supportive counseling and documentation. The MSW is a participant in a team approach to address patients' needs while providing social work services to help patients and their families cope with issues of chronic kidney disease.

Requirements

Must have a Master's in Social Work, experience managing a large caseload, plus strong communication and multi-tasking skills. Behavioral health or medical healthcare work experience is preferred. Bilingual English/Spanish candidates are welcome.
