Job Description
FT LICENSED SOCIAL WORKER Available position on a fast-paced, short term rehabilitation unit; candidates should have experience with discharge planning and completion of minimum data sets. Candidate must also have a desire and ability to interact with elderly residents and their families, have some knowledge of programs and services for the elderly, possess good organizational skills and have the ability to work with an interdisciplinary team to formulate plans of care and assist with discharge planning. Some flexibility with hours.

NOTES: Local Residents Preferred (No Relo)

Requirements
Massachusetts social worker license (min. LSW) required; LICSW supervision provided.
