Job Description

The New York Foundling offers professional opportunities and skill development in new service delivery modalities. We are committed to quality services and manageable workloads.

We currently have multiple opportunities available within our Prevention and Foster Care Programs for entry level and highly skilled professionals with a desire to make a difference in the lives of children and families.

As a Bilingual Case Planner you will provide comprehensive case management and support to children and families by: • Monitoring children’s health, school performance and living situation. • Referring birth parents to drug rehabilitative services and therapeutic services or other community resources as needed. • Scheduling and conducting office visits, home visits and sibling visits. • Provide casework counseling to foster parents, adoptive parents and birth parents. • Preparing case documentation (i.e. progress notes, psycho socials, FASP’s, and Reports of Investigation). • Attending court adoption hearings, TPR’s, trials and extension of Placement. • Attending clinical and case management meetings • Meeting all program contractual requirements and adhere to agency standards • Completing additional job related tasks as needed


Requirements
Required: • Bilingual (English/Spanish) – Must be able to communicate verbally in Spanish with Clients. • BSW only (Candidates who do not meet this educational requirement can not be considered) • Willingness to conduct field work including home visits

Preferred: • MSW only (Candidates who do not meet this educational requirement can not be considered) • Previous experience in the child welfare field • Knowledge of ACS, Connections and Promis
