Medical Social Worker (Masters of Social Work) -Lee Memorial Health System, Lee County, Florida
(Positions available in our 4 hospitals- We are expanding our services in our emergency departments)
The social work practice that occurs in hospitals and other health care settings is to facilitate good health, prevent illness, and aid physically ill patients and their families to resolve the social and psychological problems related to the illness. Medical social work also sensitizes other health care providers about the social psychological aspects of illness.
The Emergency Department Medical Social Worker provides emergent clinical interventions and crisis management, discharge planning and case management in the ED through assessment, counseling, referral and Interdisciplinary planning. The ED Medical Social Worker also expedites linking patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning.
Performs basic utilization function in identifying targeted diagnosis and provides intervention to obtain the appropriate UM admission status.
Shift : Full-time and Part-time 2:00-10:30p.m.
