Job Description
Summary of the Position:

The NFL Player Care Foundation Social Worker position plays a critical role in: identifying, assessing, coordinating services and monitoring of PCF participants who are in need of assistance. The Social Worker serves as a liaison and/or advocate between applicants and social service providers i.e. mental health therapists, government agencies, non-governmental organizations and private providers so that applicants can have their emotional, financial, housing, health, employment and other needs met. The position requires an understanding of the numerous government, non-government and private organizations that serve the underprivileged. Must have experience in connecting people with outside resources and the eligibility requirements for such programs. It requires discretion, sound independent judgment to assure that appropriate and quality services are identified and utilized.

Primary Job Duties

a. Review PCF applications and communicate with eligible participants to assess their individual and/or family emotional, financial, housing, health, employment or other needs

b. Completes intake assessment for needed services and creates an action plan of recommendations for participants.

c. Provides a written action plan summary to be reviewed and approved by PCF.

d. Identifies, implements and coordinates resources to commence action plan recommendations

e. Acts as an advocate and/or liaison between resources and PCF participants.

f. Monitors progress to assure appropriateness of identified resources

g. Prepares progress and statistical reports on PCF participants

h. Communicates verbally and in writing to PCF Board and its representatives on activities

i. Maintains records as required. May need to enter or retrieve information from a database

j. Crisis management. Social worker must be available at all times to coordinate care in crisis cases and must be able to locate appropriate local resources for crisis intervention.

k. Maintains confidentiality. Information regarding applicants is to be shared only on an as needed in establishing, implementing and coordinating applicant’s services. Any additional communication regarding applicant should be disclosed only at the direction of the PCF Executive Director.

Special Qualifications

a. Demonstrated expertise in providing excellent customer service.

b. Minimum of 5 years experience with assessing clients/participants psycho/social needs.

c. Knowledgeable of emotional, financial, housing, health and employment resources. Must have work experience in recommending and connecting PCF applicants with outside resources and understand the eligibility requirements.

d. Self-reliant and able to work remotely, with little direction. Able to work with ambiguity in a changing role.

e. Organized and detail oriented in handling of all case management.

f. Excellent communicator, including but not limited to interpersonal, telephone and written skills.

g. Excellent writing skills including drafting of action plan and case documentation.

h. Clinical or mental health work experience

i. Ability to effectively interact, identify and assess participants needs and develop an effective action plan

j. Ability to work alone and remotely from PCF office and participants. Must be disciplined and self-reliant.

Education/Training

MSW or MA in related field plus a minimum of 5 years post-Masters experience in related social service environment. Active license with no restrictions strongly preferred.

Working Conditions

The majority of the work will be performed in the home office of the social worker by telephone, email and in writing. The social worker will need to supply the office equipment such as a telephone, computer and fax machine. No less than twice a month the social worker will be required to report to the NFL PCF office in New York City.

Licensure Requirements and Malpractice Insurance.

The Social Worker will be expected to maintain, at the Social Worker’s expense, the following: (i) all professional licenses and/or certifications required for the Social Workers to provide the services described above; and (ii) professional malpractice liability insurance with policy limits not less than $1 million per occurrence and $3 million per aggregate. The Social Worker will be expected to provide PCF with evidence of such insurance in order to accept the position.

Supervision

The position is supervised by PCF Executive Director.
