LCSW, Case Manager National Naval Medical Center, Bethesda, MD
1. This task order is for the Traumatic Brain Injury initiative under the Psychological Health and Traumatic Brain Injury (PH/TBI) program. The program seeks to address gaps in the care available for the prevalent injuries among combatants from the global war on terrorism. Through adoption of risk based staffing models and a uniform, best practice standard of care across DOD, the program seeks to ensure easy, rapid access to PH and TBI care for all beneficiaries and to ensure that transition across networks of care recognizes the unique care coordination and case management requirements for PH and TBI patients.
2. Site of Service. The contractor shall provide personnel for service in the Traumatic Stress Brain Injury (TSBI) Service within the Mental Health Directorate at the National Naval Medical Center, Bethesda, MD.
3. Labor category. The contractor shall provide personnel from the following labor category:

5. Staffing and Scheduling.
a. The contractor shall provide two individual full-time Case Managers. To maintain continuity of services, each of the health care workers MUST BE A FULL-TIME INDIVIDUAL. The Commanding Officer will not approve more than one individual for each of the required positions.
b. The Department Head or a designated government representative shall supervise the health care workers.
c. Services will normally be required Monday through Friday in the Social Work Department between the hours of 0730 and 1730 for an 8.5 hour shift which includes an uncompensated .5-hour meal break. Services will not normally be required on the day of observance of Federal holidays. The contract health care workers shall be on duty for approximately 40 hours each week, except during periods of approved leave and holidays. In no instance will the health care workers be required to provide services (on-site service plus approved leave and holidays) in excess of 80 hours per 2-week period. The health care workers shall arrive for each scheduled shift in a well-rested condition and shall have had at least six hours of rest from all other Case Manager duties. d. The specific schedule for each 2-week period will be scheduled 1 month in advance by the supervisor. Any changes in the schedule shall be coordinated between the health care workers and the Government.
e. The contractor Case Managers will be credited for 8 hours worked for each holiday (if work is required on a holiday, a paid compensatory 8-hour day off will be granted).
f. The contractor Case Managers will accrue 8 hours of personal leave (annual plus sick) for each 80 hours worked. Planned absences from assigned duties shall be requested a minimum of 30 days in advance and in writing to the Commander. The health care workers shall be subject to leave approval in accordance with Section C of the basic contract. The contractor need not provide back-up coverage for the Care Managers during periods of approved leave or holidays.
6. Duties. As assigned, the health care workers shall perform the applicable duties given in Section C of the basic contract: C.10.17 CLINICAL SOCIAL WORKER
C.10.17.1 Perform a full range of clinical social worker procedures in accordance with clinical privileges granted by the commanding officer.
C.10.17.2 Practice as an interdependent member of the health team and provide important components of primary health care through direct social work services, consultation, collaboration, referral, teaching, and advocacy.
C.10.17.3 Provide direct and indirect services to both inpatients and outpatients in accordance with social work standards of practice.
C.10.17.4 Assess and treat outpatients in individual, group, couples, and family modalities exercising mature professional judgment and using a wide range of social work skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute diseases/injuries.
C.10.17.5 Conduct psychosocial assessments to determine patient needs and resources (both family support and community support). Provide counseling to patient and family in matters directly related to patients’ limitation, adjustment to medical condition, and ongoing treatment. Develop and implement discharge plans, follow-up care, and transfers to other health care facilities (e.g., nursing homes, rehabilitation hospitals, and VA facilities).
C.10.17.6 Plan and maintain referral and coordination services with civilian health and social service agencies to provide optimal patient care.
C.10.17.7 Provide consultation services to medical, nursing, and ancillary hospital staff regarding psychosocial issues, discharge plans, and follow-up care for patients and families.
C.10.17.8 Provide crisis intervention services.
C.10.17.9 Respond independently, and with various media, to appropriate military and community requests for lectures and presentations. Take the initiative in seeking out opportunities to present programs to meet the needs of service members and their families.
C.10.17.10 Consult with Naval Hospital Command and administration, supplying information and feedback regarding procedures and services provided by the Psychology Division.
C.10.17.11 Develop and maintain working relationships with community resources. Coordinate with teachers, lawyers, physicians, and representatives of their service disciplines for the benefit of the service member and their families. Take initiative in identifying and assessing the needs of the military community, and organize responses to address those needs.
C.10.17.12 Act as a human services broker, using clinical judgment and knowledge of area resources to provide information and referrals to patients and other care providers.
C.10.18 CLINICAL SOCIAL WORKER, CASE MANAGER. In addition to the applicable Clinical Social Worker duties given above:
C.10.18.1 Interface with the MTF RN Case Manager(s) in the development and implementation of the Case Management Program (CMP).
C.10.18.2 Integrate social work case management and nurse case management as a team.
C.10.18.3 Develop local strategies that use the strength and disciplines of both nursing and social work to accomplish case management.
plus the following:
a. Perform a full range of case management services on site using Government furnished supplies, facilities and equipment. The health care worker’s clinical performance will be a function of the overall demand for case management services. The health care worker will be responsible for delivery of treatment within the personnel and equipment capabilities of the MTF, provision of mandated medical surveillance and preventive services and the quality and timeliness of treatment records and reports required to document procedures performed and care provided.
b. As the primary function, provide a comprehensive range of case management services to inpatients, outpatients and their families, with substance abuse, behavioral health and brain injury issues.
c. Maintain resource information for referral to civilian agencies as appropriate.
d. Document case management interventions in accordance with command, directorate, department and professional standards of practice, utilizing command data programs Armed Forces health Longitudinal Technology Application (AHLTA) and Essentris (CIS).
e. Demonstrate Behavior Standards as described in NNMC’s Standards of Customer Service Excellence Guide specifically: Treats everyone with courtesy, compassion and respect; ensures privacy, confidentiality and dignity of others; seeks to understand and meet customer needs; takes ownership of problems. The employee should not receive more than one substantiated instance or complaint of unprofessional, inappropriate or discourteous behavior toward customers, superiors, or co-workers during the task order.
f. Follow chain of command and contributes to inter- and intra-office harmony and morale via teamwork and professional demeanor.
g. Use sound communication skills, tact, diplomacy, courtesy, and the chain of command to resolve interpersonal conflict and obtain desired outcomes.
h. Plan, coordinate, and provide comprehensive referral services.
i. Function as a member of the Traumatic Stress and Brain Injury multidisciplinary treatment team, presenting reports at team conferences and participating in the evaluation and treatment planning.
j. Plan, coordinate, and provide comprehensive referral services to other patient and family resources. The health care worker is allowed freedom of action and exercises mature professional judgment within the treatment process, dealing constructively and definitively with the problems.
k. Perform other duties, as assigned by supervisor(s).
l. Document time according to proper code on monthly MEPRS report.
m. Implement local strategies using inpatient, outpatient, onsite and telephonic CM; implement policies and protocols for home health assessments and outcome measures.
n. Implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, databases for community resources, etc.
o. Participate in multidisciplinary discharge planning and other professional staff meetings as indicated for complex patient cases and develop a database and knowledge of local community resources.
p. Identify areas for professional growth and development as related to the case manager position and maintenance of CM certification.
q. Follow cost containment/cost avoidance strategies for case management and develop mechanisms to measure its cost effectiveness.
r. Demonstrated professional knowledge gained through specialized training and experience in the application of case management services.
s. Demonstrated knowledge of internal policies and procedures governing the delivery of social services.
t. Demonstrated knowledge of program objectives, evaluations, diagnoses, and treatment of substance abuse, behavioral health and traumatic brain injury patients and their families.
u. Demonstrated knowledge of military and civilian resources that relate to the delivery of services to patients and their families.
v. Demonstrated knowledge of domestic violence and substance abuse.
